Advance Directives

In California, an Advance Health Care Directive combines two important documents, (1) a Health Care Power of Attorney and (2) a Living Will.

This document allows a named agent to make medical decisions, sign consents and releases with hospitals and doctors, and grants access to your personal medical information. It also acts as a Living Will regarding your end-of-life decisions.
